Office Forms 

For your convenience, print and complete the new patient forms and bring them with you on your first visit to our office.  At check-in you will also be asked for ID and any applicable insurance information.

►Every new patient needs to complete the first two forms below for their first office visit.

2. REQUIRED HIPAA Information Form

►Only Contact Lens patients need to also bring the third form.

3. CONTACT LENS PATIENTS ONLY Contact Lens Testing Responsibility Form
